Admissions Forms
Applicants and students of Georgia Piedmont Tech will need the following forms to process various requests. It is strongly recommended that you keep a copy of your requests for your personal records, especially those submitted via email. When emailing requests, your Georgia Piedmont Tech email should be used to prevent a false spam flagging. All requests submitted to the Admissions office will be processed within 3-5 business days unless otherwise noted. If you have any further questions, please contact the Office of Admissions.

To request an official copy of your placement test scores that you received from an assessment instrument administered by Georgia Piedmont Tech, complete the Request for Placement Test Scores form. Application Verification Request | Future Students | Admission Documents | Click here to submit this form electronically. Applicants who need to verify that an application for admission has been submitted to Georgia Piedmont Tech must complete and email the Application Verification Request form to the Office of Admissions. Once submitted, the Office of Admissions can either mail the document to the requested institution or hold it to be picked up by the applicant. |
Applicant Petition for Georgia Residence Classification | Current Students, Future Students | Admission Documents | Click here to submit this form electronically. Applicants who need to prove Georgia residency for instate tuition consideration must complete the Applicant Petition for Georgia Residence Classification form and supporting documents. Petitions must be approved before the first official day of the semester to be effective that semester. For the list of documents that be used to prove residency, click here. |

GED Transcript Request | Adult Education, Future Students | Admission Documents, GED, Transfer and Transient Student Documents | Students, who passed the GED Test in the State of Georgia, may request their test scores using the GED Official Transcript/Diploma Request form.
Applicants who took the GED Test outside the State of Georgia can request their transcript from the American Council on Education (ACE). Please be aware that there is a fee required to obtain a copy of an official transcript through ACE. |
High School/Post-Secondary Transcript Request | Future Students | Admission Documents | Applicants to Georgia Piedmont Tech must submit official transcripts from the last high school attended showing the date of graduation, or submit official GED transcripts and official transcripts from each college or university attended. Complete the online High School/Post-Secondary Transcript Request form and print to forward to the high school/post-secondary institution. It is important to include former names to expedite the processing of your documents and the completion of your admissions file. The High School/Post-Secondary Institution may charge a processing fee for each official transcript requested by a student. Contact the institution directly for payment and submission guidelines. |
